We highly recommend that you either call your insurance company and become familiar with your health coverage. You may ask the insurance company if you are covered for custom orthotics (custom shoe inserts) or CPT L3000 x2. If you fail to bring an active insurance card to your appointment, you will be responsible to pay for the services at the time they are rendered. We make every effort to bill your insurance to cover your services however we have NO control over your personal health insurance plan details like deductible, out of pocket, and co-pay. Can I request medication refills without a follow-up appointment? We require patients to make the appropriate follow-up appointment with the doctor for any medication refills. Narcotic medication, antibiotics, and other medications with a systemic effect cannot be prescribed if the patient does not follow up for the appropriate appointment time. We try to minimize redundancy; however, your safety comes first. Therefore, it is essential to schedule a follow-up appointment to ensure that your medication regimen is appropriate for your current health status.
